You need to implement a 301 redirect. If Google (and any other inbound link) is still pointing to old filenames, you want to permanently redirect requests for the old pages to go to the new pages.

You’ll need access to your server’s .htaccess file and add something like this to it:

RedirectPermanent /old-file.html /new-file.html

… where the first file name is the old filename, the second is the page you want it to redirect to. There are lots of ways to customize that redirect, depending on what you changed when you redesigned the site.
